Delete your account

Submitting this form starts the deletion of your Align Care Hub account. After confirmation, your account is permanently deleted 30 days later. Identity-bearing data is anonymized; audit records required by HIPAA §164.530 are kept (stripped of your identity).

What happens next: we email you a link to confirm. Click that link within 24 hours to start the 30-day deletion clock. You can cancel anytime during the grace period.
